How to Transfer Your Care
Download the Records Release Form
Complete the authorization form so your previous provider, hospital, or specialist group can release records securely.
Send the Form to Your Previous Provider
Submit the completed form to your prior doctor’s office or medical facility and request that records be sent to Adams Internal Medicine.
Prepare Your Health Information
Gather your medication list, insurance card, recent lab results if available, hospital discharge paperwork, and specialist notes.
Request Your First Visit
Once your records request is underway, request an appointment so Dr. Adams can review your history and help build your care plan.
